First Visit

Welcome to those of you who are joining our family of patients! Upon arrival, you will be asked to fill out some general forms, including a health history, a registration form, and a copy of our HIPAA policy and HIPAA acknowledgment.

Download them here to save some time during your visit!

For your first visit to our office, we recommend a thorough dental cleaning by one of our hygienists, followed by an initial comprehensive exam by one of our doctors. During our exam we will discuss the state of your oral health, review oral hygiene techniques, and have an interactive conversation about your dental needs. Often, there is more than one solution to your dental issues and it is important that you understand all of the options available to you. We look forward to getting to know you and your family, and to talking with you about maintaining your oral health and expectations for your smile.

Dental Associates of Lyndhurst, OH

We respect the time you have taken out of your day to be with us, and we strive to run on time so you won't be kept waiting. We ask you to arrive promptly at your appointed time, and please check in at our front desk. Occasionally, unforeseen dental emergencies occur that must be handled immediately and we appreciate your patience at these rare times. We will do our very best to keep your scheduled appointments on time. We acknowledge that unavoidable conflicts occur from time to time in our patients' lives with their scheduled appointments. Please contact our office at least twenty-four hours in advance if you need to reschedule your appointment.

Payment Options

Our front office staff can help to make financial decisions easier for our patients, so that you can get the appropriate dental care you deserve. Before scheduling treatment, we are happy to answer any general questions you may have regarding costs, but those with more complex treatment needs may necessitate a consultation. Once a treatment plan is selected, you will have treatment costs broken down for your review and approval. We request that payment be submitted as services are rendered in order to keep treatment affordable and reduce unexpected financial concerns.

For patients with dental insurance, please bring your up-to-date insurance information to your first appointment. As insurance plans or personal information change, we ask that you keep us updated. We encourage the use of a ‘pre-determination of benefits' for any substantial dental treatment. This will allow our staff to help understand your benefits and limitations of coverage, and more accurately estimate your out of pocket expenses.
